State of Arizona, Dental Assisting Certification Requirements
Dental assistant:
In the state of Arizone, dental assistants who are working directly under the supervision of a licensed dentist are able to perform basic dental procedures.
There are no education or training requirements.
Dental assistant qualified in coronal polishing:
You must be working under the supervision of a licensed dentist.
You must pass the exam.
An Arizona coronal polishing clinical skills affidavit that has been completed by a licensed dentist, or your educator/course instructor must be submitted.
There is a $50 fee.
Licensing applications are due by the last day of the year.
Expanded function dental assistant:
You must already have the coronal polishing and the radiological proficiency certifications.
You must successfully complete a CODA accredited training course, OR
You must have proof of expanded functioning certification from another state within the last 10 years.
Successfully pass the DANBs AMP, TMP, and RF exams.
Plus, have documented proof that you have completed the restorative procedures required by the ASBDE: place, contour, and finish 20 direct restorations. And, place and cement 5 prefabricated crowns. It must have been done under the supervision of a licensed dentist and documented properly on an endorsement form.
